My Healthy Resolutions offers the same self-pay visit rates across providers for eligible appointments. Self-pay may be used if you do not have insurance, your plan is out-of-network, or you choose not to use insurance.

INITIAL VISIT

Self-pay rate for an eligible initial appointment with a My Healthy Resolutions provider.

FOLLOW-UP VISIT

$125

Self-pay rate for an eligible follow-up appointment with a My Healthy Resolutions provider.

NO-SHOW FEE

$50

Fee for missed appointments that are not canceled within the required notice period.
